　Please go to a post office to fill out an "NHI IC Card Application Form" and attach a front and back copy of your Identification or Resident Certificate and one 2"x2" 　(two inches by two inches) photo from the neck up. Send to NHI for your card to be processed, which will be delivered by registered mail to the address shown on the application form.

　If the couple is working with two salaries, they should join NHI separately at different working places. Their children join NHI with the father or mother. If only one person of a couple is working with one salary, the family should join NHI together through the employer. If the entire family is unemployed, all members of the family should join NHI at their Township office where the family is registered.

　Only if the person holds a Taiwan R.O.C. ID card or if the Ministry of Foreign Affairs certifies that the country of origin has a reciprocal agreement for this service.

　Though an alien's Taiwan spouse may receive social aid and benefits, the alien cannot apply for or receive social aid or benefits unless he/she has already obtained R.O.C. citizenship.

　Bring an ID card, original Household Register (returned once verified), personal seal and three 1"x1" (one inch by one inch) photos to the Social Affairs Section of the Township office where your household is registered.
